A Gleason-type uniqueness claim for the squared-norm measure was stated without its essential dim ℋ ≥ 3 hypothesis — false as stated at dim=2 (qubits), where Gleason's theorem itself is known to fail.
Response & evidence
The dim≥3 hypothesis was added at Appendix A §A.2 and everywhere the result is invoked (Ch. 4, Ch. 10) — corrected in v2. The qubit case itself is still not addressed anywhere in the corpus; see the related open item.
